By filling out the Application below, you certify that the information provided is truthful and complete to the best of your knowledge. You acknowledge that submitting this Application in no way guarantees that any assistance will be granted. As part of the Application screening process, the Applicant may be contacted by someone from CHH to ask additional questions, request supporting documentation, etc.
Items that do not meet our Program guidelines will not be considered. These include cell phone bills, insurance payments, deductibles, cable/satellite TV bills, credit cards, security deposits, down payments, or other such bills. Please read below for our Program limits.

Assistance for most Programs is limited to once every 6 months (if approved).
The amount of available assistance can vary from week to week depending on the number of Applicants, available resources at the time, etc.
Because we are a small nonprofit with limited funding, we have the following limits for each Program:
Rental Assistance - $300.00 limit
Utility Bill Assistance - $200.00 limit
If approved, you can receive Rental Assistance OR Utility Bill Assistance, not both.
Gift cards - $200 combined limit between gas station / grocery store gift cards (depending on availability)
If approved for Emergency short-term housing, there is a 3-night limit, and we will shop around for the best deal
